Minor Dependent Addendum

This addendum applies when an adult member adds a dependent or service recipient to the account.

1. I confirm I am the parent, guardian, custodian, or otherwise legally responsible adult for the dependent listed.
2. I accept responsibility for creating, maintaining, and updating this dependent record.
3. I confirm the dependent information and medical questionnaire are accurate to the best of my knowledge.
4. I acknowledge the dependent is not a Tat2Portal member and does not have login access, credentials, messaging, billing, a Tat@ handle, or independent account rights.
5. I understand in-person guardian consent may still be required at the time of service where applicable.
6. I agree to keep the dependent information current.
