1. Tat2Portal and This Agreement
Tat2Portal is a private, membership-based platform built for the tattoo, piercing, and body art industry. It provides continuity and operational infrastructure connecting clients and independent professionals — simplifying identity, scheduling, consent, communication, records, and payment coordination within a single membership experience.
Tat2Portal is not a tattoo or piercing provider, employer, medical provider, licensure authority, regulatory agency, or guarantor of legal compliance. It does not supervise, direct, or control procedures performed by independent professionals. All procedures are independently performed by professionals operating under their own judgment, training, and applicable law.
Participation in Tat2Portal does not create an employment, agency, partnership, franchise, or joint venture relationship between Tat2Portal and any participating member.
By creating an account or using Tat2Portal, you agree to be bound by this Membership Agreement.
2. Membership Structure
Tat2Portal is a private membership network limited to registered members. Members participate in one of the following roles:
Client Members
A Client Member is an individual who participates in Tat2Portal as a recipient of tattoo, piercing, or body art services. Client Members use Tat2Portal to communicate with professionals, schedule appointments, participate in consent workflows, and preserve continuity across sessions and interactions. Client Members are responsible for providing truthful and accurate information connected to appointments, consent, health-related disclosures, and dependent participation. Dependent participation is governed by Section 4.
Professional Members
A Professional Member is an independent tattoo, piercing, or body art professional participating in Tat2Portal as part of their independent practice. Professional Members use Tat2Portal to manage appointments, communicate with clients, coordinate services, document professional interactions, and utilize consent and workflow systems.
Professional Members remain fully independent and are solely responsible for professional judgment, procedural decisions, lawful operation, sanitation practices, licensure and permit requirements, and compliance with applicable law. Tat2Portal does not supervise, direct, or control independent professional practice.
Participating Locations
Tat2Portal supports tattoo and piercing establishments as participating locations associated with professional members. Establishments are recognized as operating locations — not as membership identities. Tat2Portal does not own, supervise, or operate establishments. Operational responsibility remains with the applicable owner, permit holder, or operator.
3. Payment Facilitation
Tat2Portal provides payment coordination infrastructure to support transactions between members. Tat2Portal is not a payment processor, merchant of record, bank, or financial institution.
Transactions for tattoo, piercing, or body art services are transactions between clients and independent professionals. Tat2Portal provides infrastructure to facilitate payment coordination but does not become a party to services performed by independent professionals. Tat2Portal does not control processor approval decisions, processor terms or policies, processor fees, payout timing, or processor availability.
Professional Members are responsible for payment processor configuration, tax obligations, refund decisions, compliance with processor requirements, and lawful financial operation.
Tat2Portal separately charges membership fees associated with platform participation. Membership fees are not payment for tattoo or piercing services.
4. Eligibility and Membership Creation
To create and maintain a Tat2Portal account, a member must:
be at least eighteen (18) years of age;
create an account for themselves or for the lawful management of a minor dependent;
provide truthful and materially accurate information;
maintain a valid membership account in good standing; and
agree to participation requirements.
Minors do not independently create Tat2Portal accounts. Where permitted by applicable law and participating professional requirements, a parent or lawful legal guardian may manage dependent participation through their own Tat2Portal account. Dependent participation may be subject to applicable law, guardian authorization, professional discretion, establishment requirements, and additional documentation.
To complete membership creation, members must acknowledge this Membership Agreement, the Tat2Portal Privacy Policy, that they are eighteen (18) years of age or older, and that the account is being created for themselves or for the lawful management of a minor dependent.
5. Member Identity and Conduct
Members agree to maintain truthful and accurate account information and to participate in Tat2Portal in a lawful, honest, and respectful manner.
Members may not:
impersonate another individual;
misrepresent credentials or licensure;
knowingly provide false information;
create deceptive or fraudulent accounts;
create duplicate accounts to bypass restrictions or manipulate platform systems;
engage in fraud or abuse of platform systems;
misuse communication tools;
harass, threaten, or exploit other members;
interfere with platform functionality; or
misuse payment or continuity systems.
Tat2Portal reserves the right to request reasonable verification where necessary to protect member safety, platform integrity, or operational trust.
Professional Members remain independently responsible for lawful professional conduct and independent practice standards.
6. Records, Continuity, and Retrieval
Tat2Portal is a continuity and retrieval platform. Its purpose is to preserve records and provide authorized retrieval pathways when normal continuity is disrupted by real-world events — including artist departures, shop closures, business failures, disasters, or loss of access to records.
Record Ownership
Professional Members own the records connected to work they independently performed. Clients hold continuity rights connected to their own body art history and consent participation. Tat2Portal does not own member records. Tat2Portal provides the infrastructure through which records are preserved and retrieved.
What Tat2Portal Preserves
Records connected to platform participation may include appointment history, consent acknowledgements, communications between members, dependent participation records, professional interactions, payment coordination, and membership activity.
Record Retention
Records are retained for two years from the date of record creation. Records remain retrievable by authorized members throughout the retention period, regardless of account standing.
Access and Retrieval
Normal access to records is available to members through standard platform tools, consistent with membership role and relationship to the record. Retrieval in recovery circumstances — including artist departure, shop closure, business failure, or similar disruptions — is subject to identity verification, relationship verification, authorization review, and audit logging. Access is not automatic or blanket. It is verified and scoped.
Dependent continuity records are retrievable by the parent or lawful legal guardian account holder through the established guardian-member relationship.
Legal Compulsion
Legal compulsion — including subpoenas, court orders, warrants, or other lawful compelled process — operates separately from ordinary member access and retrieval. Tat2Portal complies with lawful compelled disclosure as required by law. This process is not subject to membership privilege or standard retrieval procedures.
Scope Limitations
Tat2Portal does not provide unverified access to records. Tat2Portal does not permit establishments to browse professional records, permit professionals to access unrelated records, or grant access to parties outside the established membership relationship. Tat2Portal does not assume legal ownership of member records or serve as a regulated records custodian.
Tat2Portal does not replace independent professional or establishment recordkeeping obligations under applicable law. Members are responsible for the truthfulness and accuracy of information they submit.
7. Membership Access and Availability
Tat2Portal continuously evolves and improves. Features, tools, and membership experiences may change as the platform grows. Feature availability may vary based on membership role, subscription level, platform maturity, or operational limitations. Tat2Portal makes reasonable efforts to maintain system availability but does not guarantee uninterrupted access at all times.
8. Account Standing and Enforcement
Membership in Tat2Portal is a conditional privilege. Tat2Portal reserves the right to suspend, restrict, or disable membership where reasonably necessary to protect member safety, platform integrity, operational continuity, lawful operation, or community trust.
Violations of this Agreement, fraud, misuse, deceptive conduct, harassment, or conduct that materially undermines platform trust may result in account action. Where appropriate, Tat2Portal may apply warnings, remediation opportunities, or graduated enforcement measures before taking action on an account.
Accounts subject to suspension or disabling are not deleted. Records associated with a suspended or disabled account are retained and remain retrievable in accordance with the retention terms in Section 6.
9. Privacy and Information Use
Tat2Portal collects, stores, organizes, and protects information connected to participation within the network. Use of Tat2Portal is also governed by the Tat2Portal Privacy Policy, which explains what information is collected, how it is used, continuity and preservation practices, communication practices, security measures, and member privacy expectations.
Members agree to review and comply with the Tat2Portal Privacy Policy.
10. Limitation of Responsibility
Tat2Portal provides membership infrastructure and continuity systems. Tat2Portal is not responsible for independent professional decisions, tattoo or piercing outcomes, establishment practices, sanitation practices, legal compliance failures of professionals or establishments, disputes between members, payment processor failures, or professional conduct outside Tat2Portal’s control.
Members understand that body art services involve independent decisions between professionals and clients. Tat2Portal does not supervise, guarantee, or validate those decisions.
11. Updates to This Agreement
Tat2Portal may update this Membership Agreement as the platform evolves. Where material updates occur, Tat2Portal will make reasonable efforts to notify participating members. Continued use of Tat2Portal following updates constitutes acceptance of revised terms.
12. Governing Law
This Agreement is governed by applicable law in the jurisdiction in which Tat2Portal operates unless otherwise required by law. Any unenforceable provision of this Agreement will not invalidate the remaining provisions.

1. What This Is
This form confirms that you understand the risks of receiving a tattoo, body piercing, or related body art procedure and that you are choosing to move forward of your own free will. It is not a procedure-specific consent — it is a platform-level agreement that governs your participation as a client member of the Tat2Portal network for 365 days from the date of acknowledgement.
Tat2Portal connects clients with independent professional members. Tat2Portal is not a tattoo or piercing provider and is not a party to any procedure performed by an independent professional. All services are agreed upon and performed exclusively between the client member and the professional member.
If you are acknowledging this form on behalf of a minor dependent, refer to the Minor Dependent Addendum, which governs dependent participation and applies in addition to this form.
2. Risk Acknowledgement
I acknowledge that I have been given the full opportunity to ask any questions I may have about obtaining a tattoo or body piercing, and that all of my questions have been answered to my satisfaction.
I acknowledge that it is not reasonably possible for my Professional to determine whether I might have an allergic reaction to the pigments, materials, or processes used. I agree to accept the risk that such a reaction is possible.
I acknowledge that infection is always a possibility following a tattoo or body piercing, particularly if I do not follow proper aftercare instructions. I understand that all possible risks cannot be fully enumerated, and I agree to accept all risks — including unforeseeable risks — arising from my decision to receive body art services through a professional member of the Tat2Portal network.
3. Tattoo-Specific Disclosures
I understand that variations in color and design may exist between the tattoo as selected and as ultimately applied to my body. I understand that on darker skin tones, colors may appear less vivid than on lighter skin tones.
I understand that future skin treatments, laser hair removal, plastic surgery, or other skin-altering procedures may result in adverse changes to my tattoo.
I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me regarding the ability to later change, alter, or remove it.
I accept full responsibility for the spelling of any proper names, words, quotes, or lettering in any language requested for my tattoo. I agree to provide accurate spelling and to carefully proof read all text before the tattoo is applied.
4. Conduct and Commitment
I agree not to attend appointments under the influence of alcohol or drugs. I understand that the independent professional retains full discretion to decline to perform services if they determine that my condition presents a risk.
I have truthfully represented to my Professional that receiving this service is my choice alone. I confirm that I do not have a physical, mental, or medical condition that I have failed to disclose that might affect my wellbeing as a result of receiving a tattoo or body piercing. My complete medical history has been provided separately through the required Medical Questionnaire.
5. Aftercare
I confirm that I have received aftercare instructions and agree to follow them throughout the healing process. I understand that any touch-up work required due to my own failure to follow aftercare instructions will be at my own expense. Touch-up terms, including any complimentary touch-up policy, are agreed upon between the client and the Professional. Tat2Portal is not involved in any aspect of procedures or touch-up arrangements.
6. Intellectual Property
I agree that the original tattoo design is the intellectual property of the Professional. I consent to the application of the procedure and to any actions or conduct of the Professional reasonably necessary to perform it.
7. Photo and Video Release
I grant the Professional member permission to photograph and/or record video of the work performed and, where applicable, my likeness, in connection with services received through the Tat2Portal network. This permission covers use for portfolio display, promotion, advertisement, education, instructional purposes, and commercial or institutional use in any medium, including the internet and any print or electronic format.
I acknowledge that photographs and videos of work performed by the Professional are the property of the Professional. I will make no monetary or other claim against the Professional or Tat2Portal for the use of any photograph or video acquired in connection with services I received.
I agree, on behalf of myself and my heirs, assigns, and legal representatives, to hold the Professional and Tat2Portal harmless from all damages, claims, actions, judgments, litigation costs, attorneys’ fees, and all other costs and expenses arising from any use of my likeness or any photograph or video acquired by or used by the Professional in connection with services performed through the Tat2Portal network.
This photo and video release is a mandatory component of this consent agreement and applies for the full 365-day term.
8. Cancellation and Fees
If I choose not to proceed with a scheduled tattoo or piercing appointment, I understand that I may be responsible for fees determined by the Professional. These may include fees for intellectual property developed at my request, opened or unused equipment, restocking costs, or loss of deposit. All such fees are set and governed by the Professional, not by Tat2Portal.
9. Location, Records, and Compliance
Services performed through the Tat2Portal network take place at a licensed tattoo or piercing studio associated with the session. Each session record captures the professional member, the client member, the date of service, the licensed tattoo or piercing studio, the materials used, and the consent packet connected to that session.
10. Liability Waiver and Hold Harmless
I agree not to bring any claim or legal action against Tat2Portal, any professional member of the network, or any licensed tattoo or piercing studio associated with a session in connection with any damages, injury, or loss — including death — arising from my decision to receive tattoo or body piercing services at any time, whether or not caused by any negligence of Tat2Portal, its members, or its representatives.
I agree, on behalf of myself and my heirs, assigns, and legal representatives, to hold Tat2Portal, its members, its representatives, and the licensed tattoo or piercing studio associated with any session harmless from all damages, claims, actions, judgments, litigation costs, attorneys’ fees, and all other costs and expenses arising from my decision to receive body art services through a professional member of the Tat2Portal network.
These waivers are intended to protect Tat2Portal, all professional members, and all licensed tattoo or piercing studios associated with sessions conducted through the Tat2Portal network.
11. Binding Agreement
By acknowledging this form, I confirm that the information I have provided is true and accurate to the best of my knowledge. I understand that this is a binding agreement and that I will be bound to its terms for 365 days from the date of acknowledgement. This form must be completed in full in order to participate as a client member of Tat2Portal.
